I just heard that not every applicant for the Dutch passport gets approval. What possible reasons that make the rejection of Dutch nationality?

It is extremely rare, in my experience. The reason is that you apply in person at the town hall of the gemeente where you live. The civil servant from the gemeente is expected to check everything over for you right there, on the spot, before taking your application fee and forwarding your application to the IND. As such, it would be surprising, to say the least, if your application was rejected by the IND. That would only happen if something came to light for the IND during its processing that could not have been apparent to the civil servant from the gemeente-- like the existence of a criminal record.

I've just checked with city hall. The lady printed out my profile in there system. I was told that the only thing I need is to my language dimploma. It seems no fine was ever recorded. I don't even know what kind of charges apply to the fine. I was ever asked to return money to 'zorgtoeslag' which turned out to be overpaid to me. I guess that was not counted as a fine.
It can't just be any fine-- you have to have been convicted of a misdrijf in the last four years (the more serious class of crimes in Dutch law: murder, rape, but also destruction of property and drunk driving, for instance), or have accepted a fine to avoid going to court for that misdrijf. Then your application for naturalization will be rejected.
